That's exactly why I'm pessimistic with the co-op, unless there is a choice or something at the end, not everyone will be able to experience the full game since you'll basically be playing two different games.
Just to throw into your point with the death of either protagonist, what sees fit to me is if there's some sort of sacrifice type thing going on or something final stand where only some people could survive, but not everyone with Carver being the one to die. The reason, I believe his backstory is something where his family was butchered by the necromorphs or something and he's only in this to destroy the thing that killed his family, or the source in this case. So basically, I think the ending will play out with Isaac finding Ellie, while shits going down, Carver says how we need to get out of here, blah-blah, there's no way out, Isaac decides if he can make a right, or save someone, it has to be Ellie since there has always been that little slice of love going on Carver does the heroic thing, touches Isaacs shoulder, “No, you actually have something worth living for…” Epic line that will live on for the ages, Isaac wants to fight Carver but Carver talks him out of it because Ellie is something Isaac did right or was able to save, unlike Nicole, then Carver throws in another epic line to signify why you should cry, “When the time comes, we'll meet again” then something to give some remembrance to his family, Isaac leaves with Ellie, Carver does either a last stand act or has to touch some panel, BANG! “We've lost so much, but there's time to rebuild all of that,” another epic line from Isaac while he holds Ellie.

Vice-versa, Isaac is finally able to rid himself of his demons if he decides to do the whole sacrifice ending type thing with Carver says, “You don't have to, there's always another way,” with Isaac replying, “You know there's not.” Carver having finally realized Isaac isn't a bad guy and how much of a dick he's been throughout the entire game decides to give his apologies, “I'm sorr-” Isaac interupting him to the usual, “There's not enough time, you've got to go!” Pushes him away and while Carver looks back witnessing Isaac watching the sun set, with Carver saying, “I'm sorry.” “Just GO!” replys Isaac. Carver flies away, Isaac dies some sort of death, and the universe seemingly saved from the necromorph threat with the death of a hero.
Since ending #2 doesn't mention Ellie, she either died during the course of the game or her fate is never resolved which with either one, her “journey” or “experience” throughout the game will be sold as a $9.99 DLC “expansion”.
